EFT Hold/Cancellation

EFT Hold Policy
Holds may be placed on your EFT Membership for up to 3 months due to health related reasons (i.e. acute illness, surgeries, maternity leave). Holds can not be placed in weekly increments. Holds are not valid for vacation or travel. Holds can NOT be used for longer than 3 months.

To put your EFT on hold we MUST have a completed EFT hold form AND a medical note signed by your doctor in our possession by the 15th of the month PRIOR to the month you would like your account to go on hold. Phone and e-mail holds are not accepted.



EFT Cancellation Policy
To cancel your EFT Membership we MUST have a completed EFT cancellation form by the 15th of the month PRIOR to the month that you would like to cancel. Phone and e-mail cancellations are not accepted. Cancellations will NOT be processed unless the adjustment difference is received.

Example: If you would like to cancel or put your membership on hold for February 1st then we must have a completed form in our possession no later than January 15th.

NOTE: We will be unable to refund any monies if the EFT Hold or Cancellation from was not received by the 15th of the month. Please refer to your EFT agreement for more information. You must have had two month's deduction taken from your account before you may cancel or be placed on hold.
